New Adult Patients
We are glad you have made the decision to join us at Surf City Dental Family and Cosmetic Dentistry! A first visit to the practice includes a comprehensive dental examination and a Full mouth series of x-rays. Both the dental exam and the x-rays will allow Dr. Parr to assess your overall dental health. The x-rays that are taken of your mouth enable Dr. Parr to view the structure of your jaw, the position of any teeth that have not yet come in, any malformed roots or other structural deformity, and any latent tooth decay. During the oral exam, Dr. Parr will check the health of your mouth – teeth, gums, cheeks, and tongue. He will evaluate the integrity of any existing dental work and make sure that it is not showing any signs of repair.
The initial visit also involves getting your medical history. When you share your medical history with Dr. Parr and his team, please be sure to provide complete, up-to-date information on your health. Inform us if you have experienced recent hospitalization or surgery, or if you have recently been ill. Please be sure to notify Dr. Parr if you have had any joint replacement, the names, doses, and frequency of any medications you are taking – whether prescription or over-the-counter products — and the name of your primary physician. This information will help Dr. Parr and his team to select the most safe and effective course of treatment for you.
As a convenience to our patients, our Health History Form and HIPAA Privacy Form are available to be downloaded below. You can save time at your initial visit by downloading the forms, printing them out, and completing those documents. Please remember to bring them to your initial visit, along with a list of any medications you are currently taking.
We look forward to seeing you at Surf City Dental!
New Child Patients
We are glad that you chose us for your family’s dental needs. Dr. Parr recommends a child’s first visit to be once the very first tooth erupted. The first dental visit is usually short and involves very little treatment. This visit gives your child an opportunity to meet Dr. Parr and our team in a non-threatening and friendly way. We may ask the parent to sit in the dental chair and hold their child during the examination. We may also ask the parent to wait in the reception area during part of the visit so that a relationship can be built between your child and our team.
During the examination, Dr. Parr will check all of your child’s existing teeth for decay, examine your child’s bite, and look for any potential problems with the gums, jaw, and oral tissues. If indicated, Shannon will clean any teeth and assess the need for fluoride. Shannon will also educate parents about oral health care basics for children and discuss dental developmental issues and answer any questions.
- Topics Dr. Parr and his team may discuss with you might include:
- Good oral hygiene practices for your child’s teeth and gums
- Cavity prevention
- Fluoride needs
- Oral habits (thumb sucking, tongue thrusting, lip sucking)
- Developmental milestones
- Proper nutrition
- Schedule of dental checkup visits
At Surf City Dental we like to see children every 6 months to build up the child’s comfort and confidence level in visiting the dentist, to monitor the development of the teeth, and to promptly treat any developing problems.
We had a great first visit! The staff was very warm and welcoming. All of my kids said they liked it there. We look forward to our next routine check up.
It has been a great experience. Dr. Parr is a great dentist and he has a great staff. I would recommend them to everyone.
