New Adult Patients
We are glad you have made the decision to join us at Surf City Dental Family and Cosmetic Dentistry. A first visit to the practice includes a comprehensive dental examination and a full mouth series of x-rays. Both the dental exam and the x-rays will allow our dentists to assess your overall dental health. The x-rays that are taken of your mouth enable them to view the structure of your jaw, the position of any teeth that have not yet come in, any malformed roots or other structural deformity, and any latent tooth decay. During the oral exam, our dental team will check the health of your mouth – teeth, gums, cheeks, and tongue. They will evaluate the integrity of any existing dental work and make sure that it is not showing any signs of repair.
The initial visit also involves gathering your medical history. When you share your medical history with our dentists and team, please be sure to provide complete, up-to-date information on your health. Inform us if you have experienced recent hospitalization or surgery, or if you have recently been ill. Also, please be sure to tell us if you have had any joint replacement, the names, doses, and frequency of any medications you are taking – whether prescription or over-the-counter products – and the name of your primary physician. This information will help us select the safest and effective course of treatment for you.
As a convenience to our patients, our Health History Form and HIPAA Privacy Form are available to be downloaded below. You can save time at your initial visit by downloading the forms, printing them out, and completing those documents. Please remember to bring them to your initial visit, along with a list of any medications you are currently taking.

New Child Patients
We are glad that you chose us for your family’s dental needs. Dr. Parr recommends a child’s first visit to be once the very first tooth erupted.The first dental visit is usually short and involves very little treatment. This visit gives your child an opportunity to meet our dentists and team in a non-threatening and friendly way. We may ask the parent to sit in the dental chair and hold their child during the examination. We may also ask the parent to wait in the reception area during part of the visit so that a relationship can be built between your child and our team.
During the examination, our dentists will check all of your child’s existing teeth for decay, examine your child’s bite, and look for any potential problems with the gums, jaw, and oral tissues. If indicated, we will clean any teeth and assess the need for fluoride. Our team will also educate parents about oral health care basics for children and discuss dental developmental issues and answer any questions.
